How early should I arrive for my first appointment?
To make the process easier, we email you the paperwork for you to fill out on our new system Jane! If you cannot complete the paperwork ahead of time, please arrive 20 minutes early.
What should I wear?
I know I need to bring my new patient paperwork, what else do I need to bring?
We also need you to bring any documents that your healthcare provider may have given to you, such as a prescription, referral form, and radiology reports. If you are using crutches, braces, or any other medical aids, please bring them with you as well. You will also need to bring your insurance cards and a photo ID.
What can I expect at my first appointment?
Is physical therapy covered by my insurance?
The simple answer is, “yes.” In most cases, Gray Physical Therapy is considered an “in-network” provider and can bill your insurance carrier. If you are not sure if your insurance will cover your treatment with us, please feel free to give us a call and we will help you make that determination. Once you have scheduled your first visit with us our staff will verify your insurance coverage so we can let you know what to expect.
What insurance does Gray Physical Therapy accept?
Cancellation Policy
Please call to cancel or reschedule your appointment 24 hours before your appointment. There will be a $35 charge for missed appointments.
